Geriatric Nurse Practitioner Program

The Geriatric Nurse Practitioner (GNP) program provides medical assessments and management of acute or chronic illnesses in skilled nursing facilities, assisted living facilities, adult family homes, and for home-bound patients in Kittitas County. The GNP program works in collaboration with the patient's primary care provider.

The role of the GNP includes ongoing alliance and communication with physicians, family, staff, and community agencies for special care needs and continuity of care.



GNP services include:

  • Completes admission assessments to nursing facilities as needed.
  • Manages day-to-day acute or chronic conditions.
  • Orders diagnostic tests to diagnose and treat.
  • Communicates with the patient's family or facility staff as indicated.
  • Collaborates with the primary physician to report medical findings and follow up, and follows orders given by the physician.
  • Obtains lab specimens.
  • Consults with the primary physician to report medical findings and develop treatment plan.
  • Teaches caregivers/family members care of the illness or disease process.
  • Manages medications to alleviate symptoms, pain, or to manage acute or chronic conditions.

The GNP program operates through Kittitas Valley Internal Medicine. Kittitas Valley Internal Medicine will bill any insurance that you have provided to the program. The statement you receive with your balance will have Kittitas Valley Internal Medicine's facility name on it. Please call 509-925-6100 with any billing questions.

How can I request this service?

The GNP service is available to residents living in skilled nursing facilities, assisted living facilities, and your primary care provider. If you have not been referred to this service but would like to be, please call us at 509-925-8483.

How often will I be seen by a GNP?

When you are in need of a health care provider at the facility, staff will inform the GNP. Monthly reviews will also be done to reconcile medication(s), address any additional concerns, and assist providers in delivering quality health care. Updates are given to family members by staff of the skilled nursing facilities or assisted living facilities. The GNPs service these facilities weekly.
