Geriatric Nurse Practitioner Services

The Geriatric Nurse Practitioner (GNP) Program works in collaboration with the patient's primary physician. The program provides medical assessments and management of acute or chronic illnesses in skilled nursing facilities, assisted living facilities, adult family homes, and for homebound patients in Kittitas County. The role of the GNP includes ongoing collaboration and communication with physicians, family, staff, and community agencies for special care needs and continuity of care.


Inpatient services include:

  • Admission assessments to nursing facilities.
  • Day-to-day management of acute or chronic diagnosis.
  • Management of medications to alleviate symptoms, pain, or to manage acute or chronic diagnosis.
  • Ordering diagnostic tests to diagnose and treat.
  • Communicates with the patient's family or facility staff as indicated.
  • Teaches facility staff or family details of the diagnoses or treatment.
  • Collaborates with the primary physician to report medical findings and follow up, and follows orders given by the physician.

Eligibility requirements:

  • Patient must reside in a nursing home, assisted living facility, adult family home, or is a homebound resident of Kittitas County.
  • Patient must be under the care of a primary physician in Kittitas County who has signed an agreement with the GNP Program.


Home setting services include:

  • Assessment of chronic or acute health problems.
  • Medication review and management.
  • Ordering diagnostic tests to diagnose and treat.
  • Draw/obtain lab specimens.
  • Consult with the primary physician to report medical findings and develop treatment plan.
  • Teaching caregivers/family members care of the illness or disease process.

Eligibility requirements:

  • Patient must be referred by a physician who has signed an agreement with the GNP Program.
  • Patient must be homebound.

Reimbursement is made through Medicare, Medicaid, Private Insurance, or Private Pay.
