We thought you might be interested in the indirect compensation the hospital provides for employees in addition to the annual salary, so we've provided a summary of our benefits. Please note that insurance coverage takes effect on the 1st day of the month following your date of hire. If the date of hire is the first working day of the month, coverage begins immediately. (LTD takes effect after the 1st day of the month following 6 months of employment). Registered Nurses who choose the per diem option (15% additional pay in lieu of benefits) are not eligible for benefits and may only change to a benefit eligible status once per year during the open enrollment period.
Due to the complexity of the following programs, this information is intended to be a summary only. Complete and specific details on each program may be obtained through the Human Resources Department. Please note there are some differences between non-contract and union contract employees. MEDICAL INSURANCE
MEDICAL INSURANCE is provided through the PEBB (Health Care Authority of Washington State). Please contact the Human Resources Department for more details. The hospital pays all the premiums for full-time employees and the premiums for part-time employees are prorated. Employees who have a 0.6 FTE or greater are eligible to participate in the Medical Insurance. Employees may elect to subscribe for dependent coverage at their own expense. Dependent coverage is partially subsidized by the hospital.
DENTAL INSURANCE
DENTAL INSURANCE is provided through the Washington Hospital Insurance Trust. There are two options, a core and buy up plan. The buy up option offers an additional $500 toward the annual maximum benefit, and an orthodontia benefit that pays 50% up to a maximum of $2,000. Please contact the Human Resources Department for more details. The hospital pays all the premiums for full-time employees and the premiums for part-time employees are prorated. Employees who have a 0.6 FTE or greater are eligible to participate in the Dental Insurance. Employees may elect to subscribe for dependent coverage at their own expense.
LIFE AND ACCIDENTAL DEATH AND DISMEMBERMENT INSURANCE
LIFE AND ACCIDENTAL DEATH AND DISMEMBERMENT INSURANCE is provided through Standard. The hospital pays all the premiums for full-time employees and the premiums for part-time employees are prorated. Employees who have a 0.6 FTE or greater are eligible to participate in the Life and AD&D Insurance. Per Diem employees are not eligible for Life and AD&D. The benefit amount is $25,000 for life insurance and $25,000 for AD&D. Employees may elect additional Voluntary AD&D and are responsible for those premiums. The additional Voluntary AD&D requires a minimum of $25,000 coverage and has a maximum of $250,000.
LONG TERM DISABILITY INSURANCE
LONG TERM DISABILITY INSURANCE is provided through Lincoln Financial. The hospital pays the premium for all employees 30 hours (0.75 FTE) per week or more. The Long Term Disability Insurance takes effect on the 1st day of the month following six (6) months of employment. The base plan provides 60% of the last month's salary up to a maximum monthly benefit of $6,600 after a ninety (90) day waiting period. LTD does not apply to part time employees with an FTE under 0.75 or per diem employees.
RETIREMENT PLAN
To provide security when you retire, the hospital has established a retirement plan. This plan has been initiated to assist you in the financial planning of your retirement. All full-time and part-time employees working at least 1,000 hours per year who are 21 years of age or over are eligible for participation on the first of the month following 24 months of employment. The hospital contributes 7% of the employee's monthly gross pay, and 10% of the employee's monthly gross pay for physicians. Employees are 25% vested after 2 years, and 100% vested after 5 years. The pension (retirement plan) shall not apply to per diem employees. Additionally all employees are eligible to participate in our Deferred Compensation Program, allowing tax benefits to those who participate.
EMPLOYEE ASSISTANCE PROGRAM
The Employee Assistance Program is provided through APS Healthcare. Hospital employees and their eligible family members (any family members residing in the household) are eligible to receive EAP services. The EAP is a confidential assessment and referral service specially designed to assist you and your family in handling personal problems.

offers voluntary short term disability, cancer care, accident and critical illness insurance through Allstate and AFLAC.
PAID TIME OFF (PTO) - NON CONTRACT & MANAGEMENT
Paid Time Off provides paid time off for vacation, holidays and short term illness to full-time and part-time non-contract and management employees. PTO does not apply to per diem employees. After completion of 90 days of employment, employees shall be eligible to take accrued PTO according to the following schedules:

VACATION ACCRUAL AND LEAVE - UNION CONTRACT EMPLOYEES
Vacation Accrual provides paid vacation to full-time and part-time employees. Vacation Accrual does not apply to per diem employees. After completion of 6 months of employment, employees shall be eligible to take accrued Vacation Leave according to the following schedule:

HOLIDAYS - UNION CONTRACT EMPLOYEES

recognizes 8 holidays and also provides for an additional floating holiday. The Holidays recognized are: New Years Day, President's Day, Memorial Day, Independence Day, Labor Day, Veterans Day, Thanksgiving Day, and Christmas Day. Part-time employees accrue holiday time based upon their FTE. Per Diem employees do not accrue holiday time. Employees may choose to save their holiday and accrue up to 48 hours per year of holiday time.
EXTENDED ILLNESS BANK - NON CONTRACT & MANAGEMENT
The Extended Illness Bank provides Non-contract employees, Exempt Directors, Managers and Administrators with compensation for extended illness or injury and can be accessed immediately upon hospitalization or after 24 hours of Paid Time Off. Extended Illness Bank time is accrued at 6 days per year.
SICK LEAVE
SICK LEAVE provides all eligible employees with compensation for illness and/or injury. Sick Leave shall not apply to per diem employees. In all cases Sick Leave shall only be payable for regularly scheduled days of work. Employees shall accrue 1 day each month (pro-rated for part-time employees). Employees are eligible to utilize accrued sick leave after completion of their 90-day orientation period.

offers a pre-tax savings program for daycare and medical expenses. Request for Reimbursement Forms can be accessed through the Manley Services website or can be obtained from Human Resources.

provides over $300 worth of test and examinations free of charge if used at KVCH. Employees can also use a $50.00 coupon for payment towards any medical care (excluding pharmacy and supplies) bills at KVCH.
BICYCLE COMMUTER BENEFIT

offers reimbursement for reasonable expenses related to bicycle commuting. The Hospital will reimburse up to $20.00 for every month the employee regularly uses their bicycle to commute to work.
CELL PHONE DISCOUNT
KVCH employees are eligible for a 15% discount from AT & T, a 22%-23% discount from Nextel/Sprint and a 22% discount from Verizon. Employee should contact AT & T, Sprint/Nextel or Verizon directly if interested.
