Home Health & Hospice

Recognizing that patient care is sometimes best provided at home, KVCH Home Health programs serve nearly 300 patients every month. Services include:
Home Health, a Medicare-certified program accepting patients with various reimbursement sources.

  • Physician-prescribed treatments
  • Skilled assessments, observations and evaluations by registered nurses, social workers
  • Physical, speech and occupational therapists.
  • 24-hour, seven-days-a-week, on-call nursing staff, including weekends and holidays
  • Home health aides to assist with bathing personal care
  • Medication management, teaching and supervision
  • Specialized teaching for dietary, cardiac, diabetic, ostomy and wound management
  • Telehealth Monitoring, a system that allows the health care team to provide continuous remote medical supervision and monitoring, with the patients involvement.

Home Respiratory Services

Home Respiratory Services provides home oxygen and respiratory equipment. Home oxygen patients are assessed regularly by our respiratory therapists. Services provided include:

  • Oxygen Concentrators
  • Portable Oxygen Systems
  • Pulmoaides
  • Home Suction
  • Home Oximetry
  • Sleep Apnea Support

Home Infusion Service

Home Infusion is a home care program that allows people to remain at home while receiving various forms of medication or hydration through veins, stomach or other tissue. Nurses evaluate each individual's needs to see if the prescribed therapy meets home care criteria.

Some of the therapies provided are:

  • Intravenous Antibiotics
  • Pain Management
  • Chemotherapy
  • Hydration
  • Nutritional Therapy
  • Central Line Management

Hospice

Hospice care is a special way of caring for a patient with a terminal illness. While the patient no longer receives curative treatment, close medical and supportive care is provided with the emphasis on making the most of each day. The hospice philosophy uses a team approach to coordinate care including the patient, family, nurses, nurses’ aides, social workers, physicians, clergy and/or volunteers. Hospice is Medicare approved.

Hospice services include:

  • Medical equipment and supplies
  • Outpatient drugs for symptom management and pain relief
  • Short-term inpatient care, including respite care
  • Physical therapy, occupational therapy and speech therapy
  • 24-hour, seven-days-a-week on-call nurses.
  • Bereavement counseling for family members.
