Organ Donation

A Donor Pledge states your desire to donate your organs and/or tissues after death. Donation lets you give another person the chance for a better life. The pledge is most helpful if you and your family have discussed the possibility of donation.

The pledge must be signed, dated and witnessed.  In addition, your next of kin will be asked to sign a consent form for donation after your death.
